I'm working on a plug-in that implements a server-based translation provider. When the plug-in is loaded, I can create a new project and select the translation provider but
I can see only the path like this-> Unknown:Servertm:///?tmuri= http://TMname,user,password but when the enabled-checkbox is checked shows this message:could not be opened and has been disabled,

Please can you provide a stack trace (the details in the error message and not just the title) or log files for the error. There is insufficient information in your post to be able to answer this properly.

While debugging my Plug-in (in c#), everything goes well until this message.I also set some breakpoints and seems to be returning correct values.

The URI in the screenshot looks ‘interesting’. Initial thought is which classes in the TM API are you using to create the server based TM provider. A break point in the ITranslationMemory.HasPermission implementation would be a starting point. Is it called? Is it returning true?
