Reason for question:
issue of images bleeding off the page in publication
no guarantee that @scale_fit = yes will restrict the height of the container element (therefore height of image)
how does Publication Manager determine where to enter page breaks into a publication?
This is not determined by Publication Manager, this is determined by the publishing stylesheets that are used. The DITA-OT (open toolkit) is used to publish to the different output formats like PDF.
