I think the default setting for the 'addFormatAttributeToReferences' parameter in the Trisoft.ContentImporter.config file should be 'false'. My colleague imported the DITA file without realizing that the addFormatAttributeToReferences parameter was set to true. As a result, the format attribute was added to the image elements, and invalid DITA files were imported into the Tridion Docs repository.
