Unable to deactivate or reset license Trados Freelance Suit 2015 after resettting Window 10 Pro. Please help.

I have my Window 10 reformatted, but I am having issues resetting my Trados License. I understand that a support request needs to be filed, but I don't see where to file a request.

Please help so I can resume my Trados 2015 asap.
