Deactivation error

Good evening, Im contacting you because I am having issues deactivating 2 accounts. The computers where those accounts were being used broke down and for some reason the online deactivation function is not working.  I wrote down the information about the accounts, and the deactivation requests below.   If you need any other information to help me deactivate those two accounts, Ill be checking this post.
