Extra Activation feature won't work

Question

I have a Studio 2011 Freelance Plus license, with Extra Activation option. The software was thus installed on 2 machines. One of the machines cracked, I was unable to return the license from it, so I manually returned the license from the second machine. After reinstalling Windows on the first machine, I activated the license, but I can no longer use it on the second machine, it keeps telling me that is is still valid on another machine. 
 In a nutshell, although I have an Extra Activation option, which gives me 2 licenses (1 + 1) to use on two machines, I can only use the license on one machine and, what's even strange, my SDL account says that both licenses are in use and there are 0 available.

Hi Bogdan, 
 You can reset your own license by using the Self Service option in your MySDL account for SDL Trados 2011 and SDL Trados Studio 2014. 
 Go to KB article #5196 and follow the instructions.

Hi Bogdan, 
 This works for both types. The important thing is they are activation codes. So just to make sure I logged in as you and tried it... I returned the one you activated in April rather than the one you activated this morning. Now it looks like this: 
 
 I just followed the instructions on the screen and forced the return. So you now have one available.
