You don't need a special installer. You can use the regular one in your SDL account.

The LSM can be installed either on the same server or on a different one, there is no bad way to go here. Even if you have it on the same server as the client application it would still result in a working setup.

You can install both Studio and the LSM on any Windows Server versions that are still supported by Microsoft so any of the ones you actually mentioned is fine. For the 2012 version you would need it upgraded to the R2 service pack.

As for hardware recommendations you can use any recent Intel Xeon  multi-core technology processor and you would need at least 16 GBs of ram for this scenario.

Yes, Bogdan-Stefan Vadean is correct.  So the main requirement here would be a Network Professional license - single user licenses will not work.  In regards to RAM I think 16GB is a minimum and probably still a bit low in my opinion for 5-7 users - but this is something you will need to monitor on the server and ramp up.
