Problem with reactivation code

Question

Hi, 
 I didn't deactivate the sdl license before the format so now I am trying to reset it via the RWS account where it seems to still be in use ( http :// feedback . sdl . com / app / survey / response . jsp ). The problem is that I can't do it, as there is nowhere the option ''return'' or ''reset'' the license (see screenshot). I downloaded sdl, but when I enter the activation key, it shows an error message (see screenshot). 
 Could you please help me?
