This question was just asked on the DITA Users Group page, and I wanted to share it here since a lot of TDX users also use the DITA OT.
You need to send your answers to the OT team, NOT to RWS.

DITA-OT is planning a major release this year - what would you like to change?
A few years ago, I got to ask people: if you could break something small in DITA in order to make your life much better, what would it be? I got some fun and some surprising answers. Well, it's time to ask the same for DITA-OT.
The next version of DITA Open Toolkit will be a major release: 4.0. In addition to big new features, a major release gives us the opportunity to break things in a way that makes the tool better.
For example, dropping some years-old technical debt could mean that plugin developers replace several mandatory steps with a single new one. It could mean replacing confusing or inconsistent parameter names with a consistent design.
So, now is your chance: if you could make a backwards incompatible change in DITA-OT — but one that made the tool better for users or plugin developers — what would it be? Let us know here, or join our Slack at https://urldefense.com/v3/__https://slack.dita-ot.org__;!!FSOJMA!dldKQkgJP60ouOD_dJL6PQBdy81QDgkbBL7bsPykv3o1nwNZu0mJTUxEo_bgdSPgP34KmQ$ and drop your ideas in the #general channel for discussion!
Thanks,
Robert Anderson
