I think since somebody will have to give a look at your specific job in order to have a clue about what you are talking, your quickest path to a solution is to open up a ticket wtih RWS support. Especially since you mentioned that you did try to add the necessary tagging yourself.

BTW: How is ADA compliance different from PDF/UA compliance?
(because PDF/UA compliance comes out of the box with XPP)

ADA Compliance and PDF/UA compliance is the same thing. We are not getting any warnings when we are running accessibility checker of Adobe Acrobat. This warning is only populating when we are running PAC checker on the same PDF.
