I have recently updated SDL Trados Studion aswell as Multiterm. Now i can´t access multiterm to manage my terms. My PC can´t find a program to open a ".sdltb" file. When i doubleclick the Multiterm application nothing happens. When i doubleclick a termbase it asks me how i want to open the file.

Try running a repair of MultiTerm, or just open MultiTerm first and then select the SDLTB from tere.

Multiterm by itself still does not open. However, there is a "Multiterm 2020 Desktop" icon on my desktop which does open.
Is there any difference between those two files?
The "Multiterm 2019 Desktop" icon starts MultiTerm (there is no MultiTerm 2020). A "*.sdltb" file is your termbase that opens up in the MultiTerm desktop application. Not anything you'd really compare as one is an application and the other is a database.
It is probably worth making sure you have compatible versions of Studio and MultiTerm:
SDL Trados Studio 2019 SR2 - 15.2.5.2145
SDL MultiTerm 2019 - 15.0.5.56289
