Can't insert images in MultiTerm 2021

Question

I encounter unusual behavior from MultiTerm 2021 when trying to insert an image. 
 (Default layout) When I insert the image file, MT does not display the image but the filename: 
 
 When I click on the filename, MT displays what looks like folder contents, and the entry is lost: 
 
 After a few seconds, I get an "Object reference not set ..." error. 
 Am I doing anything wrong?

 I'm not sure how this is happening... I tested quickly with an image at all different levels and it seems to work ok: 
 
 Is this a new termbase? 
 Did you make sure the field is a media field? 
 Have you tried recreating the termbase? 
 Not sure where to look for this to be honest.
