The segment appears as translated, but when the client opens it is not translated

Question

I received a normal package from a client, with several source segments locked already translated, and only some segments that were reviewed to be translated again.
I opened the package, translated the segments, the green tick showing that the segment was translated appears, but when I save the document, the files do not update to 100% translated, and when I create the return package and my client opens it, appears as nothing was translated.
Any idea what could cause this issue and how to solve it?

I'm struggling with this one... can't even think of an unlikely scenario that would cause this! Can you share the full filename and extension of the package you were sent, and the package you returned? Can you share the packages themselves perhaps? You can send them to pfilkin@sdl.com if this is an acceptable option.
