Is it possible to have a TM segment translation replaced and not added upon confirming a different translation for the same segment?? How??

This question has been asked for years over at least 4 versions of Trados, but never received an answer. 
 Please explain how can I avoid end up having double translations for the same segments in my TM when I am just correcting wrongly translated segments. 
 This is essential for the usability of Trados!

This has been discussed a little recently and we have found two reasons for it so far: 
 
1. Using AnyTM - we will resolve this 
2. The segments seem to contain different context 
 
In your case you mention they are the same segments. Can you provide some sample files to demonstrate this or a step by step guide we can follow to achieve this effect. If I try to do this I don't get duplicates, they are overwritten, so there must be something specific to the files, the TM, or the way you are working. 
 
I'd be very happy to work on this with you so we can get to the bottom of the problem, we can also take a Skype call to look at this together if it will be easier?
