TM was partially updated even though all the translated segments were confirmed. Any thoughts?

Question

As we're working with packages and run the TM update automatically, we noticed that one of the two segments confirmed from the bilingual file was not imported into the TM. We checked the project template, segment status and looked for existent terms in the TM. Not clear answer.

I guess I'd start with the obvious and make sure I was looking at the right TM. So are you using a Project TM? Have you added a TM at a different level to the project? (All Language Pairs when the projects TMs are at the specific language pair level... or vice versa). Have you checked the TM to be updated in your Project Settings? 
 
Perhaps this will help flesh out the thought process a little and you'll find the reason.
