Is there a way to know in which document the result from a concordance search appears?
Only if you have used the appropriate meta-data stamped to your translation units in your TM. For example, you create a field in your TM called "Document" and then add the filename of the document every time you confirm a segment. This takes some managing and is explained here:

You can automate this process by using a plugin from the appstore:

The "Record Source TU" app can automatically recognise the document name and stamp it to your TUs when translating. This way you don't need to remember to change your project settings every time you tackle a new file for translation.
