I add a file based TM to the list of TM's included in a project, and tick the "update" box in project settings -> translation memories, but the TM is nevertheless not updated. I have added the file based TM to a few projects, nevertheless only the input from the very first project to which it was added is actually in the TM. Please help.

Is this the first time that you encountered this issue? or has this happened with other TMs in other projects?
I'm curious if you can provide more context - and screenshots - that show clearly the TM not being updated - so for instance you can have the cursor in a segment that's been already confirmed, so I can see the Translation window results. Also, in your Project Settings>all languages >translation memory and automated translation. Check the TM if it has the "update" option ticked.
Also, open the TM in the Translation Memory view on the bottom left of the navigation bar> Click "open" - make sure the TM was not already added in the list, on the left side. If it's there, simply select it and remove it. Then browse for it and open it again and check the last segments when they were added.
Other possible concerns that need checking:
1. How do you update the TM, with Ctrl+Enter (on the fly at segment level) or using the batch task>update main TM?
2. Is the TM yours, locally created or was sent in a project package by your clients?
3. What happens if you create a new TM and add it to the project, does it save the segments?
