Pre-translate doesn't populate all 100% matches

Question

I have pre-translated a file in PC Studio 2014 SP2, using Batch Tasks. 
 When I open the file in Editor view, some of the segments ARE pre-translated (I only want 100% matches or Context Matches). I click on a segment that is NOT pre-translated, it auto-populates with a 100% translation from the TM. I am using only 1 TM, there are no multiple translations for this particular segment. 
 Has anyone else had this issue?

If you are able to reproduce this consistently with a small file, just as a test, it would be very helpful. I have not seen this yet, but if you can reproduce it we clearly need to address it.
