Translation Memory : It's populated with all the different revisions

Question

I have a sdlixff file showing 244 segments. The TM shows twice as much if not more. What can I do to ensure I have the exact same confirmed segments in my TM as I have in my translated document. 
 During the translation, I encountered a problem. Everytime I confirmed a segment, it showed translated, then jumping to the second one, the segment before was coming back to "not translated" and so on ... 
 Any help will be appreciated. (preferably the steps to correct the problem). Thank you

I'm struggling to guess what you might be doing here... you could be adding all your revisions to your TM as a new translation every time, you might be looking at a different TM to the one you think you're working with... many apologies as I don't know how experienced you are with this tool but this is a completely new problem for me so I'm guessing. 
 
I don't have any guess for the problem related to segments changing from translated to not translated...
