I have a docx file from the States (actually several) with a table of contents that has been generated in a strange manner. When you try clicking on an entry in the TOC it does jump to the corresponding page. However, if you select the whole TOC and select "update field" and select refresh complete TOC, Word complains that it can't find any entries for the TOC. If I tell it to simply refresh the page numbers that works. The documents also have an Index of Defined Terms that behaves in the same manner. Since I have several of these files I would like to be able to avoid rebuilding these TOCs and indexes by hand if possible. 

Does anyone have an idea what the client's customer in the US has done to create these tables or how I can best handle this?

I am using Studio 2015 SR2 - 12.2.5082.0
