When this happens, do you see other terms being shown in the Term Recognition window or is it empty?
I have noticed that sometimes, when there are several hits in a segment, Studio will not show all hits. However if you backspace one or two characters in the target segment, they will suddenly show up.

I have also noticed this erroneous behaviour of Studio. This is especially true when you have several hits for one word. Then Studio only shows one of them, especially when the hit is a two-word item.
So, if you have a "large dog" stored in your TB, as well as "large" and "dog" as separate items, and then you come across a "large dog" in you source text, the term recognition window shows only the two-word item, NOT the single-word ones.

Could this be verified and then marked as a bug, please?
