Could be related to keyboard shortcuts... these will be specific to Trados which is why you might have some clash here and not with another application.  I don't know what else you're running on your computer as the problem could be easier to solve in some other application, but based on you saying this:

I'd start with shortcuts that have "Ins" in there perhaps?  You can easily copy/paste your keyboard shortcuts into a text editor from here:

Then change or delete the shortcuts for now and see if it helps?  I only have these using "Ins" for example:

Auto-Insert Tags                           Ctrl+Alt+Ins

Copy All Source to Target            Alt+Shift+Ins

Copy Source to Target                 Ctrl+Ins, Alt+Ins

I think it may be something like this and is worth a try.

In fact, this problem happens even without using my keyboard. I just move my mouse to the segment, click on the position where I want to insert text, and the moment I start typing, existing text is overwritten. It's bizarre because in the task bar I see 'INS' even though I am overwriting text. It makes no sense at all.

ok... so you are just typing it in.  In this case perhaps it's worth making sure all your drivers are up to date.  Trados Studio may be using something that is conflicting with your drivers in some way.  Make sure that you are completely up to date, not just the service pack but also the cumulative updates:

Trados Studio 2021 - 16.1.5.4270
MultiTerm 2021 - 16.1.1.541

If you are, make sure your keyboard drivers are also up to date, and if you still have this problem after that I'll log a case so a support engineer takes a look at your computer. The solution here will not be anything we can address in Trados Studio.  It's going to be something associated with your computer and the software you're running so will take some time for a support engineer to investigate this with you.

I have SDL Trados Studio 2021 SR1 - 16.1.5.4270, I even changed keyboards, Trados is the only program that is giving me these problems. I will try to analyse the problem myself for the time being to see if I can reproduce the error because I don't like people taking over my computer. What I did see, however, is that each time it's getting into this unwanted insert mode, I notice a little delay. As if it wants to do something, but never follows through.
